There are cards that provide benefits that will directly, specifically, say to remove them from your deck and campaign. Otherwise everything gets put into your discard pile and reshuffled for a new draw deck.
This off course excludes cards and investigators that allow you to do hoopy stuff in getting/accessing cards from your discard deck.
